RESUMO Racional: O aumento da prevalência da obesidade levou ao aumento significativo da ocorrência de síndrome metabólica, fator de risco reconhecido para aumento da morbimortalidade por doenças cardiovasculares. A hiperglicemia ou diabetes mellitus do tipo 2, dislipidemia e hipertensão arterial são seus principais componentes. Desde 2015, diretrizes internacionais reconheceram os benefícios da cirurgia bariátrica em cada fator isolado desta síndrome. Objetivos: Avaliar o impacto do bypass gástrico em Y-de-Roux nesta síndrome comparando períodos pré e pós-operatório com análise laboratorial, e comparar a razão cintura/estatura e o IMC em relação a determinação do perfil de risco cardiometabólico. Métodos: Realizou-se um estudo retrospectivo com base prospectiva selecionando 80 pacientes submetidos à bypass gástrico em Y-de-Roux. Foram analisados o colesterol total, HDL, LDL, triglicerídeos, glicemia de jejum, hemoglobina glicada, insulina, índice de massa corpórea (IMC), vitamina D, vitamina B12, perímetro abdominal e relação cintura/estatura em três períodos: o pré-operatório de 1 a 6 meses, pós-operatório de 1 a 6 meses e pós-operatório de 1 a 2 anos. Resultados: Houve melhora em todos os parâmetros das análises clínicas. O IMC, no pré-operatório, teve a média dos valores de 39,8, no pré-operatório de 1 a 6 meses, os valores caíram para 33,2 e no pós-operatório de 1 ano média foi de 26. O perímetro abdominal teve média dos valores de 118,5, no pré-operatório, 105,2 no pós-operatório de 1 a 6 meses e 90,3 no pós-operatório de 1 a 2 anos. A relação cintura/estatura teve 0,73, 0,65 e 0,56 no pré, pós 1 a 6 meses e 1 a 2 anos respectivamente. Conclusão: O bypass gástrico em Y-de-Roux melhora a síndrome metabólica e a relação cintura/estatura é superior ao IMC na avaliação do perfil do risco cardiometabólico.

ABSTRACT Background: The increased prevalence of obesity has led to a significant increase in the occurrence of metabolic syndrome, a recognized risk factor for increased morbidity and mortality from cardiovascular diseases. Hyperglycemia or type 2 diabetes mellitus, dyslipidemia and arterial hypertension are its main components. Since 2015, international guidelines have recognized the benefits of bariatric surgery in each isolated factor of this syndrome. Aim: To evaluate the impact of Roux-en-Y gastric bypass in this syndrome comparing pre- and postoperative periods with laboratory analysis and to compare waist/height ratio and BMI in relation to the determination of the cardiometabolic risk profile. Methods: A retrospective study was carried out, selecting 80 patients undergoing Roux-en-Y gastric bypass. Total cholesterol, HDL, LDL, triglycerides, fasting glucose, glycated hemoglobin, insulin, body mass index (BMI), vitamin D, vitamin B12, waist circumference and waist/height ratio in three periods were analyzed: the preoperative period from 1 to 6 months, postoperative from 1 to 6 months and postoperative from 1 to 2 years. Results: There was an improvement in all parameters of the clinical analyses. The preoperative BMI had a mean value of 39.8, in the preoperative period from 1 to 6 months, the values dropped to 33.2 and in the postoperative period of 1 year, the mean was 26. The perimeter mean values of 118.5 preoperatively, 105.2 postoperatively from 1 to 6 months and 90.3 postoperatively from 1 to 2 years. Waist/height ratio was 0.73, 0.65 and 0.56 in pre, post 1 to 6 months and 1 to 2 years respectively. Conclusion: Roux-en-Y gastric bypass improves metabolic syndrome and waist-to-height ratio is superior to BMI in the assessment of the cardiometabolic risk profile.

Introdução: Concomitantemente com a pandemia da obesidade, observa-se fenômeno de envelhecimento global, com aumento significativo na população de idosos obesos. Objetivo: Revisar as indicações da cirurgia bariátrica para os idosos, focando principalmente a morbimortalidade dos procedimentos. Método : Revisão da literatura nas bases PubMed/Medline e Scielo com foco na relação dos fatores de risco com diversas técnicas de cirurgia bariátrica em idosos. Foram cruzados os seguintes descritores na forma de AND/OR: Obesidade; Cirurgia bariátrica; Complicações; Idosos. Conclusão: Nas pessoas com idade superior a 60 anos os procedimentos bariátricos representam opção aceitável e efetiva de tratamento. Os idosos devem ser tratados em centros especializados, de grande movimento cirúrgico e baixos índices de morbidade e mortalidade13. Com isso, eles experimentam os benefícios da cirurgia bariátrica com aceitável morbimortalidade. Contudo, a idade por si só não deve funcionar como um impeditivo absoluto na indicação cirúrgica.

Introduction : Concurrently with the pandemic obesity is observed global aging phenomenon, with a significant increase of obesity in the elderly population. Aim : To review the indications for bariatric surgery for the elderly, mainly focusing on the morbidity and mortality of procedures. Method : Review of the literature in PubMed/Medline and Scielo focusing on the relationship of risk factors with different techniques of bariatric surgery in the elderly. The following descriptors were crossed in the form of AND/OR: Obesity; Bariatric surgery; Complications; Elderly. Conclusion : In people older than 60 years bariatric procedures represent acceptable and effective treatment option. The elderly should be treated in specialized centers with experience in major surgical procedures and low morbimortality. Going in this way, they experience the benefits of bariatric surgery with acceptable morbidity and mortality. However, age alone should not be considered as an absolute impediment for surgical indication.

INTRODUÇÃO: A associação entre obesidade e doença do refluxo gastroesofágico apresenta alta incidência e pode estar presente em metade dos obesos com indicação cirúrgica. Operações bariátricas podem também induzir refluxo por si só - de modo diferente do fator IMC -, e seus mecanismos são dependentes do tipo do procedimento realizado. OBJETIVO: Efetuar revisão bibliográfica comparando os dois procedimentos atualmente mais utilizados para tratamento cirúrgico da obesidade e analisar a relação deles com o advento de doença do refluxo gastroesofágico pré-existente ou de surgimento somente no pós-operatório. MÉTODO: Foi realizada revisão bibliográfica nas bases virtuais Medline/Pubmed, Scielo, Lilacs, Embase e Cochrane cruzando os seguintes descritores MeSH: gastric bypass AND/OR anastomosis, Roux-en-Y AND/OR gastroesophageal reflux AND/OR gastroenterostomy AND/OR gastrectomy AND/OR obesity AND/OR bariatric surgery AND/OR postoperative period. Foram consideradas pertinentes 135 referências e utilizadas 30 neste artigo. Também foi adicionada a experiência dos autores deste artigo no manuseio dessas técnicas com esse mister. CONCLUSÃO: As alterações estruturais causadas pela técnica operatória na gastrectomia vertical apresenta maior comprometimento dos mecanismos anti-refluxo predispondo a indução da DRGE no pós-operatório quando comparado à técnica operatória realizada no Bypass gastrointestinal em Y-de-Roux.

BACKGROUND: The association between obesity and gastroesophageal reflux disease has a high incidence and may be present in half of obese patients with surgical indication. Bariatric operations can also induce reflux alone - differently from BMI factors - and its mechanisms are dependent on the type of procedure performed. OBJECTIVE: To perform a literature review comparing the two procedures currently most used for surgical treatment of obesity and analyze their relationship with the advent of pre-existing reflux disease or its appearance only in postoperative period. METHOD: The literature was reviewed in virtual database Medline/PubMed, SciELO, Lilacs, Embase and Cochrane crossing the following MeSH descriptors: gastric bypass AND / OR anastomosis, Roux-en-Y AND / OR gastroesophageal reflux AND / OR gastroenterostomy AND / OR gastrectomy AND / OR obesity AND / OR bariatric surgery AND / OR postoperative period. A total of 135 relevant references were considered but only 30 were used in this article. Also was added the experience of the authors of this article in handling these techniques on this field. CONCLUSION: The structural changes caused by surgical technique in vertical gastrectomy shows greater commitment of antireflux mechanisms predisposing the induction of GERD postoperatively compared to the surgical technique performed in the gastrointestinal Bypass Roux-en-Y.

INTRODUÇÃO: A técnica operatória da gastrectomia vertical ainda não foi totalmente padronizada e, por isso, existem questões a serem resolvidas. Há tendência dela ter seu uso cada vez mais frequente, uma vez que já provou ser efetiva na perda de peso, com baixa morbimortalidade e bons resultados pós-operatórios. Contudo, a doença do refluxo gastroesofágico, que pode dela resultar, ainda não está bem elucidada. OBJETIVO: Apresentar variante técnica para a gastrectomia vertical que deixa o estômago totalmente tubulizado e sem manipulação nos esfíncteres pilórico e esofágico inferior. TÉCNICA: Inicia-se a gastrectomia vertical com a ligadura dos vasos da grande curvatura tendo como referência anatômica o piloro até o ângulo esofagogástrico. O grampeamento inicia-se também a partir do piloro na mesma direção. Para modelagem é utilizada sonda de Fouchet 32 F . O grampeamento é feito justo à sonda modeladora. Finaliza-se com sobressutura da linha de grampeamento e drenagem tipo "sump". RESULTADOS: O procedimento foi utilizado em 55 pacientes portadores de IMC entre 35 e 41. As complicações nesse grupo foram: duas fístulas ao nível do ângulo esofagogástrico, uma torção do tubo gástrico e uma conversão pós-operatória para derivação gástrica em Y-de-Roux por refluxo gastroesofágico intratável com medicamentos. As fístulas foram tratadas por procedimento endoscópico com dilatação e septotomia. Disfagia leve por torção respondeu satisfatoriamente com dilatação endoscópica. O maior período de seguimento foi de dois anos. A perda do excesso de peso foi de de 67,7% em um ano e de 69,7% em dois. Os pacientes que eram portadores de diabete melito tipo 2 apresentaram controle da doença em 84,6% no primeiro ano e 91,6% no segundo. Outras comorbidades tiveram controle em todos os casos. CONCLUSÃO: A variante técnica proposta para a gastrectomia vertical deixa o estômago totalmente tubulizado, em pequeno calibre, propiciando diminuição da cavidade gástrica livre e manutenção da atividade funcional dos esfíncteres naturais. Contudo, pode levar a refluxo gastroesofágico indesejado, que precisa ser melhor mensurado em pesquisas futuras.

INTRODUCTION: The surgical technique of sleeve gastrectomy has not been fully standardized and, therefore, there are issues to be solved. There is a tendency to increase its application due to proved efficiency in weight loss, low morbidity and good postoperative results. However, gastroesophageal reflux disease, which can result from it, is still not well understood. AIM: To present variant technique for sleeve gastrectomy that leaves the stomach totally with the appearance of a homogeneous tube, without manipulation in pyloric and lower esophageal sphincters. TECNIQUE: Vertical gastrectomy starts with vessel ligation of the greater curvature at the pylorus till esophagogastric angle. Stapling also starts from the pylorus in the same direction. For modeling was used Fouchet 32 F and all the staple line was done just to it. At the end, oversuture of the staple line and "sump" drainage were done. RESULTS: The procedure was used in 55 patients with BMI between 35 and 41. Complications in this group were two fistulas at the esophagogastric angle, one twist of the gastric tube and one postoperative conversion to Roux-en-Y gastric bypass due to untreatable gastroesophageal reflux disease. Fistulas were treated by endoscopic procedure with dilation and septotomy. Mild dysphagia due to tube twisting responded satisfactorily to dilatation. The longest follow-up was two years. The loss of overweight was 67.7% at one year and 69.7% in two. Patients who were suffering from type 2 diabetes mellitus showed disease control in 84.6 % in the first year and 91.6 % in the second. Comorbidities were controlled in all cases. CONCLUSION: The variant technique proposed here for sleeve gastrectomy leaves the stomach homogeneously in a shape of a tube and in small caliber, providing small free gastric cavity and keeping natural functional activity of the sphincters. However, it may lead to unwanted gastroesophageal reflux, which needs to be better measured in future research.

OBJETIVOS: Verificar as alterações do peso e índice de massa corporal em pacientes obesos grau II e III com diabete melito tipo 2 nos períodos pré e pós-operatório e as alterações dos parâmetros laboratoriais de glicemia de jejum, glicemia pós-prandial, hemoglobina glicada, insulina nos períodos pré e pós-operatório. MÉTODOS: Realizou-se estudo prospectivo selecionando 40 pacientes com obesidade grau II e III, submetidos à derivação gastrojejunal em Y-de-Roux sem anel. Analisou-se no pré e pós-operatório de 60 dias o peso, índice de massa corporal, glicemia de jejum, glicemia pós-prandial, hemoglobina glicada e insulina. RESULTADOS: O peso médio pré-operatório foi de 107,3Kg diminuindo para 89,5Kg no pós-operatório. O índice de massa corporal médio inicial foi de 39,5Kg/m2 e 32,9Kg/m2 com 60 dias de pós-operatório. A glicemia de jejum no pré-operatório foi de 132 mg/dl e no pós-operatório diminuiu 40,4 mg/dl em média. A glicemia pós-prandial foi de 172 mg/dl no pré-operatório e 111,6 mg/dl no controle pós-operatório. A hemoglobina glicada inicial foi de 7% declinando para 5,7% no pós-operatório. A insulina pré-operatória foi 29,6 uIU/ml e a pós-operatória 13,9 uIU/ml. Todas as variáveis apresentaram significância estatística com p<0,001. CONCLUSÃO: Houve significante diminuição de peso e no índice de massa corporal entre os períodos pré e pós-operatórios e diminuição também significante dos parâmetros laboratoriais de glicemia de jejum, glicemia pós-prandial, hemoglobina glicada, insulina entre os mesmos períodos.

OBJECTIVES: 1) To assess the changes in body weight and body mass index in grade 2 and 3 obese patients with type 2 diabetes mellitus, pre and postoperatively; 2) to assess the changes in pre and postoperative fasting blood glucose, postprandial glucose, glycated hemoglobin and insulin. METHODS: A prospective study was undertaken with 40 selected patients with grade 2 and 3 obesity and type 2 diabetes mellitus, all with indication for surgical treatment of obesity. All patients underwent Roux-en-Y gastric bypass without ring. The clinical parameters weight and body mass index were analyzed, as well as the laboratory parameters fasting blood glucose, postprandial glucose, glycated hemoglobin, and insulin. The analyses were carried out in the preoperative period and at 60 days postoperatively. Statistical analysis was conducted with Student's t and the Kolmogorov-Smirnov tests. RESULTS: Mean preoperative weight was 107.3 kg, declining to 89.5 kg postoperatively. Mean initial body mass index was 39.5 kg/m², and 32.9 kg/m² at 60 postoperative days. Preoperative fasting blood glucose was 132 mg/dL, which was reduced to 40.4 mg/dL, on average, postoperatively. Postprandial blood glucose was 172 mg/dL in the preoperative period and 111.6 mg/dL in the postoperative measurement. Initial glycated hemoglobin was 7%, declining to 5.7% postoperatively. Preoperative and postoperative insulin levels were 29.6 µIU/mL and 13.9 µIU/mL, respectively. The level of significance was p<0.001 for all variables. CONCLUSION: There was a statistically significant reduction in body weight and body mass index between the pre and postoperative periods, as well as in fasting blood glucose, postprandial glucose, glycated hemoglobin.

Muitas substâncias têm sido utilizadas após hepatectomias parciais com o fim de se saber como elas atuam no processo de regeneração. Entre elas encontra-se o hormônio de crescimento. Visando conhecer a influência deste hormônio, 50 ratos Wistar, machos, com idade de 170 dias e peso médio de 380g, foram divididos em dois grupos de 25 animais, respectivamente experimento e controle. Sob anestesia inalatória de éter etílico sofreram laparotomia mediana e hepatectomia de aproximadamente 70%, ressecando-se os lobos mediano e lateral esquerdo que eram pesados e em seguida fazia-se a laparorrafia. Os ratos do grupo experimento receberam hormônio de crescimento através de injeção diária, subcutânea, de 0,4U/kg num volume de 0,12 ml. Os animais do controle receberam igual volume de água destilada pela mesma via. Sacrificados em lotes de cinco animais de cada grupo com 36, 72, 168, 240 e 336 horas. Avaliava-se o peso do animal e o peso total do fígado. Através da fórmula de Kwon et al. a regeneração hepática foi calculada. No estudo microscópico conhecia-se as figuras de mitose em 20 campos. O percentual de regeneração do peso da víscera mostrou-se maior no grupo tratado com hormônio de crescimento nas aferições de 168h (p = 0,011), 240 h (p = 0,011) e 336h (p < 0,0001). Nos animais do grupo experimento a regeneração foi completa a partir de 168h enquanto que no controle não atingira 100% com 336h. As figuras de mitose estiveram presentes no grupo experimento até 240h sendo significativamente maior no grupo experimento nas aferições de 36h (p < 0,001) e 72h (p < 0,01). Concluiu-se que o hormônio de crescimento leva à regeneração mais rápida do fígado após hepatectomia, em ratos.

Many substances have been used after partial hepatectomy in order to determine their effect on the regeneration process. One of them is growth hormone. We divided 50 male Wistar rats aged 170 days and weighing on average 380g into two groups of 25 animals each (experimental and control groups) to determine the effect of this growth hormone. The animals were anesthetized with ethyl ether and submitted to median laparotomy and a 70% hepatectomy, by medium and left lateral lobes resection. The removed lobes were weighed and the abdomen closed. The rats in the experimental group received growth hormone by daily subcutaneus injection of a 0,4 U/Kg dose, in a volume of 0,12 ml. Control animals received an equal volume of distilled water, injected by the same route. Five animals from each groups were then sacrificed after 36, 72, 168, 240 and 336 hours, and the weight of each animal and of the liver was determined. Hepatic regeneration was calculated by the formula of Kwon et al. and mitotic figures observed in 20 histological fields by light microscopy. Percentage weight of the regenerated liver was higher in the animals treated with growth hormone at 168 h (p=0,011), 250 h (p=0,011) and 336 (p<0,0001). In the experimental group, regeneration was complete starting at 168 h, whereas in controls did not reached 100% after 336 h. Mitotic figures were present in the experimental group up to 240 hs, and were present in significantly higher numbers in the experimental group at 36 h (p<0,001) and 72 h (p<0,01). We conclude that growth hormone leads to a more rapid regeneration of the liver after hepatectomy in rats.

Investiga-se a interferência do hormônio de crescimento no processo de cicatrização cutânea de ratos. Utilizaram-se 50 ratos machos com 170 dias de idade divididos em 2 grupos: controle (n=25) e experimento (n=25). Submeteram-se os ratos dos dois grupos à laparotomia mediana de 4 centímetros sob anestesia inalatória de éter etílico. Realizou-se a laparorrafia com 2 planos de sutura, o primeiro peritônio-músculo-aponevrótico e o segundo o da pele com chuleio contínuo de fio monofilamentar de náilon 4.0. Os animais do grupo experimento receberam 0,4U/kg/dia de hormônio de crescimento por via subcutânea diluído em 0,12 ml de água destilada e os do grupo controle recebiam igual volume do veículo pela mesma via. Realizaram-se as aferições após 36, 72, 168, 240 e 336 horas, ressecando-se retalhos da parede abdominal contendo as cicatrizes, que após preparação histológica e tratamento pelo Sirius red, forneceram a concentração do colágeno. O percentual da área de cicatriz ocupada por colágeno foi maior no grupo experimento nas 36 horas (p=0,0106), 72 horas (p=0,0089, 168 horas (p=0,0149) e 336 horas (p=0,0116). O percentual de colágeno I foi semelhante nos dois grupos nas 36 e 72 horas e maior no grupo experimento nas 168 horas (p=0,0218), 240 horas (p=0,0067) e nas 336 horas (p=0,0027). O percentual de colágeno III apareceu em proporção semelhante nos 2 grupos nas 36 e 72 horas. Sua proporção foi maior no grupo controle nas 168 horas (p=0,0216), 240 horas (p=0,0067) e 336 horas (p=0,0027). Estes dados permitiram concluir que o hormônio de crescimento levou a maior concentração de colágeno e acelerou a maturação das cicatrizes.

The aim of the study was to evaluate the role of growth hormone in the skin healing repair . Fifty male rats, with a mean age of 170 days were allocated in 2 groups of 25 animals: the control and experimental groups . Under ether anesthesia, all the animals underwent to a 4 cm abdominal midline incision . The laparotomy was sutured in 2 layers ( first layer- peritoneum, muscle and aponeurosis and second layer- skin) with uninterrupted 4-0 nylon suture. A dosage of 0.4U/Kg/day of growth hormone, diluted in 0.12 ml of distilled water was given subcutaneously to the animals of the experimental group and the animals of the control group received the same amount of distilled water only. Samples of abdominal wall scar were taken 36, 72, 168, 240 and 336 hours after the initial procedure. The collagen concentration was evaluated after histological preparation with Red-Sirius dye. The percentage of the area of the wound occupied by collagen was greater in the experimental group at 36 hours (p=0.0106), 72 hours (p=0.0089), 168 hours (p=0.0149) and 336 hours (p=0.0116). The percentage of collagen I was similar in both groups at 36 and 72 hours and greater in the experimental group at 168 hours (p=0.0218), 240 hours (p=0.0067) and 336 hours (p=0.0027). The percentage of collagen III was similar in both groups at 36 and 72 hours and it was greater at 168 hours (p=0.0216), 240 hours (p=0.0067) and 336 hours (p=0.0027). We can conclude that growth hormone induced greater collagen deposition and maturation in the wound.
